Enrollment Forms
- 4-H New Member Enrollment in Jefferson County: Youth who enroll in the Jefferson County 4-H program must complete the 2-page enrollment form, the fee form and the 1-page behavior expectation form (member agreement). Enrollment is open to any youth in 5-year-old Kindergarten to one-year past High School, forms can be submitted any time during the year to the 4-H club leader who submits it to the Jefferson County UWEX Office. To exhibit at the County Fair in July enrollment forms must be submitted prior to March 1st.
- Re-enrolling 4-H members need to return their completed enrollment forms by the due date specified by the club organizational leader, and to the County UWEX Office before November 1st.
Adult Volunteers enrolling in Jefferson County 4-H
- Re-enrolling adult volunteers will receive forms from the club organizational leaders in the fall of the year. Re-enrolling volunteers need to return their completed forms before November 1st.
- New adult volunteers to the Jefferson County UWEX 4-H program must attend an orientation training program session. This orientation program will be held November 16, 2011 (7 p.m.) at the UWEX Office, 864 Collins Rd., Jefferson. The session will take 1.5 hours of time. Please call Judy at 920-674-7440 to register.
- OFFICER TRAINING: Club Officer Training is held at the U.W. Extension on September 29th from 6:30-8:30 p.m. All officers should be in attendance.

$20 Jefferson County fee – payable to Jefferson County
$5 Leader’s Association fee – payable to Jefferson County Leader’s Association
